Subjective cognitive complaints in premenopausal women: an overlooked clinical reality demanding recognition

Abstract

Subjective cognitive complaints (SCC)—self-perceived declines in memory, attention, and executive function—are a frequently dismissed clinical reality for premenopausal women. While often occurring before detectable deficits appear on objective neuropsychological tests, these symptoms represent a significant neurobiological and occupational challenge. SCC is driven by genuine neurobiological changes resulting from fluctuating reproductive hormones, primarily estradiol. Estradiol serves as a critical neuroprotective agent that enhances synaptic plasticity, modulates neurotransmitters (acetylcholine, dopamine, and norepinephrine), and supports mitochondrial biogenesis. The premenopausal transition acts as a "neuroplasticity window" where hormonal variability disrupts synaptic scaffolding and alters functional connectivity within the executive control network. Research indicates a profound "invisible productivity crisis," with approximately 25% of working women underperforming due to these symptoms. Nearly 86% of women in this transition report "brain fog," leading to an estimated $1.8 billion in annual lost working time in the U.S. alone. Furthermore, SCC with preserved objective performance is now recognized as a potential Stage 1 risk marker for preclinical Alzheimer’s disease. SCC should be treated as a legitimate clinical entity rather than being reflexively attributed to psychological distress. Recommendations include routine clinical screening for women aged 35 and older, the development of SCC-specific assessment tools, and the implementation of menopause-inclusive workplace policies to mitigate career disruption and systemic gender inequity.
